One theme may host more than one student. If you offer several different themes, please submit a separate form. Each form becomes its own entry in the List of Placements (LOP) that students choose from. Your company name and website, together with sections 2 and 3, are published in the LOP and read by students. Fields marked "internal" are not published.
Please complete this form in English. The text you write here is published to students as you write it.
1つの研修テーマで複数名の学生を受け入れていただくことも可能です。複数の異なる研修テーマがある場合は、テーマごとに別々の応募用紙をご提出ください。各応募用紙が、学生が応募先を選ぶ研修受入先一覧（List of Placements、以下「LOP」）の1件として掲載されます。貴社名およびウェブサイト、ならびにセクション2・3の内容は、LOPに掲載され、学生が閲覧します。「internal（非公開）」と記載された項目は掲載されません。
本用紙は英語でご記入ください。ご記入いただいた文章は、そのまま学生向けに掲載されます。
